This form is signed by the buyer only before sale.


A buyer’s guide must be posted by the dealer on the side window of every vehicle displayed for sale with both sides visible.

The label may be removed temporarily for a test drive but must be re-affixed after the test drive is complete.


The dealer shall have the buyer’s signatures.


The original or a copy of the buyer’s guide, including all terms and required disclosures, shall be presented to the buyer upon completion of a used vehicle sale.

This document should be used to identify ALL known defects in writing. If you disclose it, you do not have to fix it, excluding Division 12 items.
